AgeWell PACE

The recently designed Agewell PACE website connects individuals seeking care for their elderly family members with a compassionate community of healthcare professionals. These professionals provide person-centered care, enabling older adults to comfortably reside in their own homes.

AgeWell PACE is a non-profit that addresses the health and social needs of older adults with complex needs in Marin and Sonoma Counties. It was founded in 2021 by two federally qualified health centers, LifeLong Medical Care, and Santa Rosa Community Health. AgeWell PACE opened its first center in Rohnert Park in 2023, providing wraparound care including a skilled team, adult day health care, home care, a full specialty provider network, prescription drug coverage, and more. The program focuses on prevention, wellness, and enhancing the quality of life through care and support.

The Problem

Exygy partnered with Agewell Pace in early 2023 to build a new community-facing website. Often the first touch point people have with an organization, it is vital that an organization’s website is thoughtfully designed, informative, easy to navigate, and accessible especially when the organization is providing life-transforming services like Agewell PACE. Working within the project's budget and time constraints, Exygy began not only developing a website that would serve Agewell PACE but also laid the foundation for more custom builds for small non-profits to come.

A New Solution

Fundamental to the work we do at Exygy is the belief that digital tools (websites, apps, portals, etc) have the potential to improve lives and strengthen communities. Custom-built digital tools, however, can often be costly and time-consuming to produce. Unfortunately, this price tag can exclude small organizations (with small budgets) that still need a custom site that serves their important needs.

This problem got us thinking. How might we still create custom sites but lower the barrier to entry?

In our partnership with Agewell PACE, we began crafting a solution: a custom CMS built by Exygy that can flexibly adapt and be implemented for a variety of non-profits and small organizations.

Design and Build


We followed a similar approach to all our other engagements. First, we held a product workshop to align with key stakeholders on organizational and user needs. We then hosted a branding workshop to define the site's visual design, UI, and photography direction. From there we designed templates to be handed off to our engineering team for the build.

At the same time, we were able to create multipurpose design and engineering elements that would coalesce to form a lasting CMS template. This upfront work will allow us, later on, to minimize the number of new feature builds while still having all the necessary building blocks to adapt the CMS to the specific needs of any future organization.

The Impact

Launched in the summer of 2023, the Agewell Pace website is currently serving the communities surrounding aging adults in the Bay Area. The modern and accessible site informs and enrolls individuals to get the care they need.
